All transcript requests received by Friday, December 20, 2013 will be processed before the winter break closing. All requests received after that time will be processed when the college reopens.

- Full Name (both current and any previous names)
- Current mailing address
- Tri-C ID number or social security number
- Date of Birth
- Dates of Attendance
- Number of Copies requested
- Complete name and address of person or office receiving the transcript
- Your Signature
- Your Phone Number
Fee Schedule: $5.00 per transcript

Credit Card (Master Card, Visa, Discover and American Express) information must include type of card, account number, expiration date, name of card holder as it appears on the card and signature of the card holder.
Privacy and Financial Requirements
In accordance with the Federal Family Educational Rights & Privacy Act (FERPA) of 1974 and subsequent amendments, your academic records cannot be released without your consent. All outstanding obligations (financial, academic or administrative) to the College must be cleared before a transcript request can be processed.
