PROGRAM APPLICATION

TRANSCRIPTS

To have Cuyahoga Community College evaluate your transcripts for course transfer and equivalency, you must:

1.     apply to the college (available online), receive a student number

2.       send your transcripts to:

               Cuyahoga Community College

               Enrollment Processing Center

               P.O. Box 5966 Cleveland, Ohio 44101

3.       Contact the Counseling Center at the campus of your choice to schedule an appointment with a  counselor, AFTER you have received notification from the Enrollment Processing Center that your transfer credit evaluation has been completed.

The PA Program staff does NOT interpret transcripts. You may contact the specific department supervisor (Bio, Chem, etc...) for course evaluation of specific courses in question. 

HOW DO I APPLY?

Cuyahoga Community College Physician Assistant Program utilizes the Central Application Service for Physician Assistants (CASPA). CASPA is a convenient, web-based application service that allows an applicant to apply to several participating PA educational programs by completing a single application.
