Bicycle Safety Day 2020 Form

I grant permission to Cuyahoga Community College District (the “College”) and its trustees, officers, students, vendors, consultants, agents and employees (collectively, the “affiliates”) to use my name, photograph, video, likeness, voice or statements in any and all manner and media in connection with publicity, advertising, promotion, publication and any other purposes throughout the world, in perpetuity. I waive any right that I may have to inspect or approve any such use. I understand that if I choose not to grant this permission, it is my responsibility to inform photographers, videographers or other relevant affiliates of the College of my desire not to be recorded.

Release: In consideration of your acceptance of this entry, I herby for myself, my heirs, executors and administrators, waive and release any and all rights and claims for damages that I may have against Cuyahoga Community College and all sponsors or the cities in which the Bicycle Safety and Awareness Day is instructed, their representatives, successors and assignees for any and all injuries suffered by the participant in said Bicycle Safety and Awareness Day.
